As mentioned this patient had recurrent UTI. Sonographic imaging of the urinary system was done. Both kidneys are small, show gross dilatation of the pelvicalyceal system and ureter. Renal parenchyma is echogenic, grossly reduced in thickness. Now, you should get the final picture. This patient has chronic renal failure and bony changes of renal osteodystrophy.

This is an add-on case for osteoporosis. The skeletal survey shows a very classical appearance of marble bone disease. Sclerosis is evident in the skull bones, especially involving the base as well as mandible mandibular angle is obtuse. Anterior fontanelle is open. Long bones show combination of longitudinal striations as well as horizontal growth disturbance phenomena. Bone within the bone is very well shown in the tarsal bones. In the chest x-ray bone sclerosis is evident and the clavicles are well-developed. Unfortunately, the hand radiograph is missing. In this context Pycnodysostosis is an additional possibility. Radiograph of the hands, wherein ostial lysis of the distal phalanx is shown, are essential for making the diagnosis.
